DESCRIPTION:New Bern Area Chamber of Commerce cordially invites you to attend the Ground Breaking Ceremony to celebrate the new location of Carolina SeniorCare -East \n\n\n\nCarolina SeniorCare is a non-profit provider of PACE services and is sponsored by EveryAge\, a Newton\, NC-based\, not-for-profit Christian ministry that has offered senior housing and services for over 50 years. \n\nPACE (Program of All-Inclusive Care for the Elderly) is a provider-based health plan that supports participants with access to medical and supportive services they need without moving from their home. As a model of care\, PACE has been in existence for over 35 years throughout the United States. It is available to those 55 and older with chronic care needs who reside in a specific geographic area and can live safely at home with PACE services and have Medicare\, and/or Medicaid and/or private funds. Some participants may have a monthly patient liability which is determined by the County Department of Social Services. Those under 100% of the poverty rate have no co-pay.\n\nCarolina SeniorCare is bringing PACE services to the New Bern and surrounding area with office construction underway at Neuse Road in New Bern. The program focuses on prevention and wellness with a commitment to enhancing the quality of life for seniors and their families. \n\nAn enrolled participant has access to: \n\n A pharmacy and a medical clinic staffed with physicians\, physician assistants\, nurse practitioners and nurses.\n Physical\, occupational and speech therapy and a therapy gym. outfitted with equipment and modalities.\n Short-term rehab and restorative services.\n Day center.\n Transportation to and from the center and to medical appointments. \n Sensory room to decompress. \n Therapeutic recreation.\n Access to specialty medical care. \n Nursing home and hospital care if needed.\n\n \n\nAs a partner within the health care community\, Carolina SeniorCare collaborates with participants to prevent emergency room visits and re-admissions to the hospital. PACE is a support to local agencies by managing high utilizers of public emergency services.
